To help Food for Purpose provide effective Nutrition and Wellbeing services, we have to obtain, process and store personal information about our service users. This privacy policy and consent form details our obligations on how we treat your personal details. To use your personal information, Food for Purpose requires your consent. Please read through this Privacy Policy and sign the consent below if you agree to allow us to use your personal information in the way set out in this policy.

What is personal information?

Personal information is information that relates to a living individual, and allows that individual to be identified. Certain types of personal information is categorised as 'sensitive personal data', this is information which relates to racial or ethnic origin, political opinions, religious beliefs, membership of trade union, physical or mental health, sexual orientation, gender, alleged or real offences and proceeds from offences.

How do we collect information?

There are different ways that we collect information.

  1. You may provide personal information to us when communicating with us.

  2. You may communicate with us when making an enquiry or responding to an informational piece about one of our services, when requesting information about your personal health or for a quote, when making other contact in relation to yourself or a dependant, or when a member of our team have initiated a contact with you in relation to yourself, your family or your organisation.

  3. Whenever you communicate with us we may need to obtain personal details about you to enable us to perform any actions that may be required as a result of the contact. We may also confirm who you are by asking you to tell us some of your personal details, for example your date of birth.

  4. We may also obtain your personal details from organisations you have dealt with, such as your church or faith organisation. We will only request personal information that is appropriate for our business functions, and you may refuse to provide information if you think any requests are inappropriate. Please note that we may not be able to carry out Food for Purpose services effectively, if you refuse to provide information that is required.

How do we use your personal information?

The purposes for which we process your personal details are to allow us to:

  • assess your progress and evaluate results from our programmes/ consultations

  • support the management of your health

  • conduct surveys and statistics on service user profiles in order to improve services

  • conduct surveys and statistics when requested by relevant health bodies and agencies

  • provide you with information about the health of BME faith communities in general that may be of interest to you

  • and to enable us to comply with our legal and regulatory obligations.

Who may we disclose your personal details to?

We may have to provide information to national health agencies and bodies. We may be required by law to disclose your personal details in order to comply with applicable laws and lawful government requests. If a request for your information is made, we will ensure we receive your consent prior to sharing your information.
