Partnership with SEL ICB

FFP has partnered with South East London Integrated Care Board (SEL ICB) to work with local people living or at risk of diabetes from these communities - African Caribbean, South Asian, Polish, Latin American, Portuguese, and Spanish.

Our aim will be to identify concerns / issues with current diabetes service provision and to co-design with the local communities action plans that can lead to change.

We are keen to hear from you if;

• You are someone living with diabetes 

• You are a family member of someone living with diabetes 

• You are at risk of diabetes or have been diagnosed with prediabetes 

• You are from any of the following communities:

  • African Caribbean

  • South Asian

  • Polish

  • Portuguese

  • Spanish

  • Latin American
